Please quote your best offer for the item/s described herein, subject to the Terms and Conditions provided on the last page of this RFQ. Submit your quotation duly signed by you or your duly authorized representative no later than August 10, 2026, at 10:00 AM . Suppliers shall submit, together with their quotation, a copy of their valid 2026 Mayor’s/Business Permit , BIR Form 2303 (Certificate of Registration), DTI Certificate of Registration (if applicable), and a duly notarized Omnibus Sworn Statement (OSS), in accordance with Republic Act No. 12009 and its Implementing Rules and Regulations (IRR). However, pursuant to GPPB Resolution No. 10-2026, a notarized OSS shall not be required for procurement projects with an Approved Budget for the Contract (ABC) of Fifty Thousand Pesos (₱50,000.00) and below . For procurement projects with an ABC exceeding Five Hundred Thousand Pesos (₱500,000.00 ), suppliers shall also submit a copy of their latest duly filed Income Tax Return (ITR) or Business Tax Return , as applicable.
