RFQ 06-081-26 Supply and Delivery of 1,500 packs of Organic Fertilizer

The Department of Agriculture Regional Field Office IX, through its Bids and Awards Committee, intends to procure the “SUPPLY AND DELIVERY OF 1,500 PACKS OF ORGANIC FERTILIZERS" with an Approved Budget for the Contract (ABC) of One Million Five Hundred Thousand Pesos (P1,500,000.00) through Section 34 (Small Value Procurement) of the Implementing Rules and Regulations of Republic Act 12009. The Procuring Entity hereby invites all interested providers to quote their bid offer, subject to the Terms and Conditions stated herein. Please submit your duly signed Quotation Form addressed to the Bids and Awards Committee to be received by the BAC SECRETARIAT of the PROCURING ENTITY at Department of Agriculture – RSO IX, Zamboanga City , on or before August 13, 2026, 08:30am . Late quotations shall not be accepted. Interested bidders shall also submit an original copy of the eligibility requirements with the quotation form, on or before as specified above the mentioned deadline for submission: Valid Mayor’s/Business Permit Valid and updated PhilGEPS Registration Certificate (Platinum Membership) Latest Income Tax Return Registration Certificate from Securities and Exchange Commission (SEC), Department of Trade and Industry (DTI) for sole proprietorship, or Cooperative Development Authority (CDA) for cooperatives or its equivalent document. Tax Clearance Audited Financial Statements Statement of Single Largest Completed Contract (SLCC) - Supply and Delivery of Organic Fertilizers -at least 50% of the ABC. Original Notarized Omnibus Sworn Statement (OSS) Original Notarized Bid Securing Declaration Notarized Certificate of Distributorship, if applicable Certificate of Registration as Dealer/Manufacturer and Product Registration from FPA/BAFS, whichever is applicable. Organic Certificate issued by DA-BAFS Accredited Organic Certifying Body (OCB) License to Operate from Fertilizer and Pesticide Authority (FPA), if applicable. Third-Party organic certification, if applicable.
