REQUEST FOR QUOTATION RFQ 2026-08-099 The Department of Education, Division of Batanes , through its School Governance and Operations Division (SGOD) intends to procure and hereby invites suppliers who can submit their best offer for the “Procurement of Liquid Handsoap in the Implementation of WinS Program” for CY 2026 in accordance with Section 34 (Small Value Procurement) of the Implementing Rules and Regulations of Republic Act No. 12009. The Approved Budget for the Contract (ABC) to payments under the contract for the WHOLE LOT is PhP59,885.00 . Source of Fund: GAA-SPECIAL PURPOSE FUND-SBFP Evaluation Criteria: Lowest Calculated Bid (LCB) For evaluation purposes, prospective bidders are required to submit the following documents in triplicate copies: Valid Mayor’s Permit issued by the city or municipality where the principal place of business of the prospective bidder is located (for NEW suppliers/bidders); Duly accomplished Request for Quotation Form (RFQ); PhilGEPS registration number (for NEW suppliers/bidders). A valid and updated PhilGEPS Registration Certificate (Platinum Membership) (all pages) may be submitted in lieu of the Mayor’s/Business Permit; Omnibus Sworn Statement (OSS) as per NGPA GPPB Prescribed Form & Special Power of Attorney (SPA) or Secretary’s Certificate, if applicable.
