Supply and Delivery of Various ICT Equipment

The Department of Agriculture RFO-12, through its Bids and Awards Committee (BAC), intends to procure for the “Supply and Delivery of Various ICT Equipment” with an Approved Budget Cost of One Million Three Hundred Nineteen Thousand Nine Hundred Ninety Pesos (Php1,319,990.00) for FY 2026, in accordance with Section 34 (Small Value Procurement) of the Implementing Rules and Regulations of Republic Act No. 12009 or the New Government Procurement Act. Should this merit your favorable consideration, please submit your duly signed quotation addressed to the Bids & Awards Committee (BAC) for the item/s described herein, subject to the attached Annex A provided as part of this Request for Quotation (RFQ). Quotation must be duly signed by your authorized representative and submitted not later than August 17, 2026, 10:00 o’clock in the morning , at the Bids and Awards Committee, Department of Agriculture Regional Field Office 12, 2nd Floor, Annex 2 Bldg. Jose Abad Santos St., Corner Confessor St., Brgy. Zone III, Koronadal City . Opening of Quotation shall be on August 17, 2026, 1:00 o’clock in the afternoon at Department of Agriculture RFO-12 Conference Room (2nd floor), Prime Regional Government Center, Brgy. Carpenter Hill, Koronadal City . Quotations may also be submitted through the email address indicated below. Interested bidder shall also submit a copy of the following documents along with the quotation on or before the above specified deadline of submission of quotation: Mayor’s/Business Permit; PhilGEPS Registration Number/Platinu, Membership; Notarized Ominibus Sworn Statement (OSS) (GPPB-prescribed forms) with attached duly notarized Special Power of Attorney, Board/Partnership Resolution or Secretary’s Certificate, whichever is applicable; and Income/Business Tax Return (For ABC above Php500,000.00 only) The Head of the Procuring Entity (HoPE) reserves the right to reject any and all quotations, declare a failure of procurement, or not award contract in accordance with Section 70 of the IRR of RA No. 12009.
