Scaffolding PCAB License Needed: Small A (Trade/E) Contract Duration: 60 Calendar Days The Bulacan State University invites all eligible and PhilGEPS-registered suppliers/consultants to quote the best offer for the item/s described herein subject to the Terms and Conditions and within the Approved Budget for the Contract. Required Documents/Information to be submitted: 1. Accomplish this RFQ correctly and accurately. 2. Please quote your lowest price on the item/s listed below comprising the necessary fees including delivery and taxes such as VAT. 3. Quotation must be valid within 120 calendar days after the opening of bids. 4. For the scope of works not included in the Construction Specifications, Plans, and BOQ of this project additive-deductive works may apply but shall be subject to the approval of the BAC and PMO. Any additive-deductive to this project may be recommended by the PMO during the duration of the contract at its own initiative and subject to the rules of RA 12009 5. Bidders are required to submit the following additional documents: 5.1. Organizational chart for the contract to be bid indicating the List of Key Personnel; 5.2. List of contractor’s key personnel to be assigned to the contract to be bid, (with their complete qualification and experience data, including license and date of validity); 5.3. List of contractor’s major equipment units, which are owned, leased, and/or under purchase agreements, supported by proof of ownership or certification of availability of equipment from the equipment lessor/vendor for the duration of the project, as the case may be. 6. The bidder with the Lowest Calculated Quotation/ Proposal shall submit all the eligibility documents supporting its PhilGEPS Membership within five (5) calendar days together with its latest Income and Business Tax Returns filed to the BIR. (A) Manpower Schedule (B) Equipment Utilization Schedule (C) Construction Method (D) Construction Safety and Health Program (DOLE) (E) Contractor's All Risks Insurance Policy (CARI) (F) PERT / CPM Diagram (G) Construction Schedule and S-Curve Terms and Conditions: 1. All entries shall be typed or written in a clear legible manner. 2. BulSU reserves the right to reject any or all offers and accept an offer as may be considered most advantegeous to the Agency. 3. No alternate quotation/offer is allowed, suppliers who submitted more than one quotation shall be automatically disqualified. 4. Price quotations to be denominated in Philippine Peso shall include all applicable government taxes. 5. BulSU may require you to submit additional documents that will prove your legal, financial and technical capability to undertake this contract. 6. In case of tie quotations, suppliers' presence are required during tie breaking through draw lots or toss coin. 7. In case supplier's pro forma quotation is submitted, the duly signed Request for Quotation (RFQ) issued by BulSU must still be submitted, as it shall remain the governing document for all terms, conditions, and specifications. 8. Please fill up the form accordingly, state either “COMPLY” or “NOT COMPLY” in each of the individual parameters of each specification, and sign the conforme below. Failure to conform will result in a rating of “FAILED”. 9. Mode of Submission: Sealed or Electronic at Procurement office 2nd floor or at svp.procurement@bulsu.edu.ph BY LOT PURCHASED
